Questions and Answers
Quick answers for clients filling out a form and for advisors sending them.
For Clients
I got an email from Wealth Forms. Is it real?
Wealth Forms sends forms for financial advisors who use Altitude CRM. The email names your advisor and their firm. If you're unsure, call your advisor at a number you already have and ask whether they sent it. See how to check an email is real.
Who can see the information I enter?
Your advisor's firm. Your answers are stored encrypted, on your advisor's behalf, and nothing updates your records until your advisor reviews it.
Why do I need a code to open my form?
Forms ask for personal information, so a one-time Secure Form Code is sent to you by email or text before the form opens. It expires in 10 minutes and works once. Never share it. No one from Wealth Forms or your advisor's firm will ask you for it.
My code didn't arrive or expired. What do I do?
Check your spam folder, then tap "Send a new code" on the form page.
Can I stop and finish later?
Yes. Your answers save as you type. Open the same link again before it expires to pick up where you left off.
Do I need an account or an app?
No. Forms open in your web browser on a phone, tablet or computer.
Can I fill it out on paper instead?
Yes. Ask your advisor for a PDF copy. When you send it back, your answers go into the same review your advisor uses for online forms.
Is my electronic signature valid?
You agree to sign electronically before you sign, and Wealth Forms records when and how you signed along with a fingerprint of the exact form. Ask your advisor if a specific custodian needs something different.
For Advisors
How does prefill work?
When you send a form from a contact in Altitude CRM, every field that maps to the client's record is filled in before the form goes out, including household members and dependents.
Can I use my custodian's PDFs?
Yes. Upload the PDF and place fields on the page. Clients fill it online, and you download the completed copy in the original layout.
What happens when a client submits?
You get a review screen that shows each confirmed and changed answer next to what was on file. Apply it to update the record. Nothing changes until you do.
Can forms go out under my firm's brand?
Yes. Import your logo, colors and fonts, and point your own domain at Wealth Forms so links use an address your clients know.
Which email address do forms come from?
Your firm's verified sending domain if you have one, your connected mailbox, or wealthforms.com on your firm's behalf.
Can I see who has finished?
Yes. The Forms workspace shows who hasn't opened their form, who is in progress and how far along, and who has submitted.
